Every review cycle uses a questionnaire. You'll find them in Supplier reviews › Questionnaires, and they're listed on Modules & forms too.
Ready-made questionnaires
| Questionnaire | Who answers | What it covers |
|---|---|---|
| Quarterly supplier review (stakeholders) | Your stakeholders, 1 to 5 | Four sections: Service and operations (everyone asked), Finance, IT and security, and Contract owner. The questions are linked to the standard six scorecard measures. |
| Annual supplier relationship survey (stakeholders) | Your stakeholders, 1 to 10 | Trust, communication, understanding of your business, delivery, ideas, value, overall satisfaction, whether they'd recommend the supplier, and two written questions. |
| How is it working with us? (suppliers) | Supplier contacts, 1 to 5 | How quickly you pay, how clear your requirements are, communication, how easy you are to do business with, the purchase order and invoice process, whether they'd recommend working with you, and the one thing you should change. |
When an admin adds a ready-made questionnaire, the sections are given to your teams whose names fit (for example, a team called Finance gets the Finance section). Change them in the editor. Anyone running a cycle can also use a ready-made questionnaire that hasn't been added yet: it's saved with that cycle.
Add or change a questionnaire
Only admins can add or change saved questionnaires. Click Add this questionnaire on a ready-made one, Start a blank questionnaire, or Edit on one in the list. You can set:
- Who answers: your stakeholders, about a supplier, or suppliers, about working with you.
- Scale: 1 to 5, 1 to 10, red-amber-green, or yes or no. On 1 to 5 and 1 to 10 you can allow decimals, such as 3.5.
- Sections, and for a stakeholder questionnaire, which teams answer each section. No team ticked means everyone asked answers it.
- Questions, each with an answer type: a score on the scale, yes or no, pick one option, or a written answer (up to 4,000 characters). Mark questions that must be answered.
- For scored questions on a stakeholder questionnaire, a scorecard measure it counts towards, and a weight (2 counts twice as much as 1; 0 leaves it out of the combined score).
Every section also has an optional "Anything to add?" box for comments.
A questionnaire can have up to 30 sections, 60 questions in a section and 150 questions in all. A pick-one question needs at least two options.
Preview
Preview shows the questionnaire exactly as people will see it. For a stakeholder questionnaire, choose a team to see just the sections that team answers. Nothing you choose in a preview is saved.
Changing and archiving
Cycles already running keep the questions they were launched with, so changing a questionnaire never changes answers already given. Archive takes a questionnaire out of use; cycles that used it keep their results, and Bring back restores it.
