Supplier due diligence questionnaire

Questions to send suppliers, grouped by topic, with a guide to which sections each tier needs.

Templates are guides only. Adapt the questions to your own policies and risks, and check them with your legal team before use. This is not legal advice.

A supplier due diligence questionnaire asks a supplier to tell you, in writing, about the things that could hurt you if they went wrong: who owns them, whether they'll stay in business, how they protect your data, and how they treat people. The questions below are grouped by topic. Send only the sections that fit the supplier's tier and what they will do for you.

  2. Will our data be used to train any AI model, by you or by your AI provider?
  3. Will a person check AI-produced work before it reaches us?

After the answers come back

  • Check the evidence, not just the answer. A "yes" to Cyber Essentials should come with a certificate in date.
  • Record follow-up questions and the supplier's replies.
  • Decide whether any gaps are acceptable, need a contract clause, or rule the supplier out, and record who decided.
  • Set a date to ask again: certificates expire and suppliers change.

SupplierVault sends the right questions for each supplier's tier. Questionnaires are edited to match your policy, answered by the supplier through the portal on the Professional plan and above, and saved with the evidence and certificate expiry dates on the supplier's record.
